For many reasons. Here are just a few:
- You certify that all the information you provide is the truth. You answers can be used later in discovery or at trial to impeach your credibility with inconsistent statements
- The value of your case is measured, in part, by the information you provide in your answers.

3. Detailed description of the nature, extent and duration of any and all injuries? A sample answer:
I was severely injured in an auto accident. I injured my head, neck and back in this accident. I suffered a cerebral concussion with loss of consciousness; a herniated cervical disc at C4-5, with painful restriction of motion; pain radiates from my neck down to the fingers of my right hand, with numbness; a herniated lumbar disc at L5-S1; my back pain goes down my leg into my foot, with numbness;
